- Winter Sports Forms
  Athletes who intend to participate in a sport for the Winter 2016-2017 season are required to complete the following three forms: 1. Sections 1-7 of the PIAA-Comprehensive Initial Pre-Participation Evaluation. Section 6-The Physical page must be dated after 6/1/16 and signed by the Physician. Section 7 is only required if the Physical (Section 6) was certified before the earliest date in which the forms can be collected for the upcoming Winter Season (10/7/16). 2. Assumption of Risk, Student Accident, Injury and Risk and Guidelines Form. 3. ImPact Consent Form. All three forms must be completed and submitted to the Athletic Office for review in order for a student to be eligible to participate. If this is the students 2nd sport in the current school year; the student will need to complete the following: 1. Sections 1 and 7 of the PIAA Comprehensive Initial Pre-Participation Evaluation. Section 7 must be dated no earlier than 10/7/16. If any answer on Section 7 is marked off as yes-Section 8 will also be required and must be filled out by the physician. The Winter Sports Forms are due no later than Friday November 7th. All forms can be downloaded on the CHS Athletic Webpage or picked up in the Athletic Office.
- Winter Sports Meeting
  The Winter Sports Meeting will take place on Wednesday, October 26, 2016 at 7:00pm in the Auditorium.
